How to Get PhilHealth ID

The Philippine Health Insurance Corporation | PhilHealth ID is one of the important documents for identification. Upon registration as member of PhilHealth, you’ll be given a PhilHealth Number and the PhilHealth ID.

There are two types of PhilHealth ID, which are the traditional laminated ID and the digitized ID card. Regardless of the type of ID you want to avail; the first thing to do is to know how you can get a PhilHealth ID card.

Employed individuals have lesser concerns because the employer will be the one to process the registration as well as the payment of contributions. But, for those who are self-employed or unemployed, they need to register first as a “Voluntary Member” or “Individually paying Member”.

PhilHealth ID Basic requirements

  • Duly accomplished PhilHealth Member Registration Form
  • Two 1×1 photo
  • Two valid ID (government issued)

Other documents are required for those uniformed personnel of PNP, AFP, BFP and BJMP. SSS and GSIS pensioners can also apply for PhilHealth membership.

The regular PhilHealth ID (paper ID) can be used when claiming outpatient or inpatient hospital benefits. You can claim this type of ID without charge from any PhilHealth LHIO nearest to your place.

The digitized ID card or the PhilHealth Insurance ID card is issued to PhilHealth members in the formal and informal economy sector with a charge or Php90. A dependent can be declared on your ID card but the member will pay Php358 and present two valid IDs of the dependent. The payment can be done to any Bayad Center outlet.
