Guide on How to Obtain PSA Death Certificate

A death certificate is an important document that will confirm the death of an individual. This document is issued by the Philippine Statistics Office and it contains details including the name, place and date of death, cause of death and other pertinent information.

Any institution or facility that handles the remains of the person will be the one to prepare the death certificate. It will be forwarded to the Local Civil Registrar within 30 days for registration. It would take 6 months before the PSA can issue the death certificate.

The death certificate is one of the requirements when filing claims from any government agencies, settlement of properties, or from life insurance companies.

There are three ways on how you can get a copy of the death certificate issued by the PSA:

  • Online portal
  • SM business center
  • Census Serbilis center

How to apply a copy of PSA death certificate at Census Serbilis Center

If you need to get the death certificate on the same day, you can visit any Census Serbilis Center near you and do the following:

  • Ask for an application form and fill it out. Make sure to use black pen only.
  • Pay the processing fee that costs Php140 at the cashier.
  • Wait for the given schedule of releasing the PSA death certificate

As much as possible, you should visit the outlet as early as possible particularly during business days to avoid the crowd and you can get the copy on the same day.

How to request PSA death certificate through online application

If you have access to the internet, the easiest way to request for PSA death certificate is through e-Census. It is the official online portal of PSA in partnership with Unisys Philippines.

Here’s a quick guide:

  • Visit the e-Census website

Read the information on how to get PSA death certificate online and click the “Click here to request now” button.

  • Accept the terms and conditions

After making your request, you’ll be redirected to another page which is the terms and conditions. Make sure to read first the fine lines and click the “I accept” button if you agree to the terms.

  • Accomplish the contact and delivery information form

Since you’re requesting for PSA death certificate online, you need to provide your contact and delivery information including your name, contact number, address, and email address. Click the “Next” button once done. Make sure that all the details are correct, and then click the “Confirm” button.

  • Click the box indicating the death certificate

The request summary will displayed and you will see the box that indicates “death certificate”, click the button to make your request.

  • Answer some confirmation questions

Before processing your request, you need to answer a couple of confirmation questions. After answering the questions, you can click the “Next” button.

  • Input the details pertaining to the deceased person

In this portion, you need to ensure that all the information you’ll provide are true and accurate. Indicate if how many copies you will request and the purpose. Click “Save” and “Confirm” button.

  • Submit your request

If your request is displayed in the request summary page, click “Submit” button.

  • Payment and delivery

The last step is the payment for your request. You can pay by using a credit card, Bayad Center, UnionBank, and Banco de Oro. The processing fee is Php300 per copy. You will receive the copy within 9 working days.

How to apply PSA death certificate at SM

SM Business Center has been providing services including request for civil documents. You can request for a copy of PSA death certificate from any SM Business Center. You can claim the copy within 6 business days or have it delivered right at your home.
